I have been researching the civil records for Rivisondoli and have copies of documents that have the D'Altorio names. There are seven D'Altorio's in my family tree, so far. In looking at the original manifest for your grandfather Igino D'Altorio from 16 Sept 1921, it shows a wife he left in Rivisondoli as Clotilde but gives no surname for her, so I was unable to look for her arrival in the states.
